To enter the parking garage, continue up the hill to the stop sign at the top of the hill. Turn right onto 124thAve. NE Go to the 4 way stop and turn right into the Evergreen Surgery and Physician Center parking garage. Go down 1 level and park on level “D”. Enter the Surgery and Physician Center through the set of double tan doors on level D of the parking garage.
Dr. Faw’s office is the first office suite on the right – Suite 440.
Heading South on I-405
Traveling South on I-405, take Exit 20 (NE 124th St.) Turn left at the signal onto NE 124th St. (You will pass over I-405). Turn left at the 3rd signal light onto Totem Lake Blvd. Go to the next light at 120th Ave NE and turn right. Continue up the hill and past Evergreen Hospital. Turn right onto NE 130th Lane (stop light). Continue up the hill (past the emergency entrance) to the stop sign at the top of the hill. Turn right onto 124th Ave. NE Go to the 4 way stop and turn right into the Evergreen Surgery and Physician Center parking garage. Go down 1 level and park on level “D”. Heading North on I-405
Take Exit 20-B (NE 124th St./Totem Lake Blvd). This exit will split into a "Y". Stay in the right hand lane and take the Totem Lake Blvd exit. . At the Stop light, continue going straight, heading up 120th. Continue up the hill and past Evergreen Hospital. Turn right onto NE 130th Lane (stop light). Continue up the hill (past the emergency entrance) to the stop sign at the top of the hill. Turn right onto 124th Ave. NE Go to the 4 way stop and turn right into the Evergreen Surgery and Physician Center parking garage. Go down 1 level and park on level “D”.
